My doctors use 75630 and 75716 or 75710 together. We obviously get denials as the latter codes being inclusive to the 75630. The doctor is asking if the 75630 includes bilateral run-offs and states that if it does not these codes should be used together and paid separately. I don't know the answer to that.... can anyone give me some help?

angiograms

A non-selective pelvic study is assigned code 75630 or 75716 depending upon the physician dictation of vessels studied.

75630 is an abdominal angiogram plus bilateral iliofemoral lower extremity (run-off).

75716 is pelvic angiogram, extremity, bilateral.

75710 describes S&I for extremity unilateral.

Another scenario:
Catheter placed in abdominal aorta, contrast injected for imaging of the aorta, then the catheter is repositioned in lower aorta (usually at, or just above, the bifurcation) with a study of the lower extremities. IF, the physician documents full and complete exams of both components, assign the following codes: 36200, 75625, 75716.

Could you possibly describe what is meant by 'run-off' so I can fully understand this scenario?

Run Off

The most common way to perform abdominal aortography and bilateral lower
extremity run-off (contrast runs into lower extremitie(s) is with two catheter placements. For example, the catheter is advanced from the common femoral access to the suprarenal aorta, and abdominal aortography is performed.
The catheter is then repositioned at the aortic bifurcation, and lower extremity
run-off (contrast injected and runs into lower limb(s) is performed.
The aforementioned describes full and complete evaluations of the abdominal aortography and lower extremity angiography. These two separate and identifiable examinations are reported with the following codes:
75625 Aortography, abdominal, by serialography, radiological S&I and 75716 Angiography, extremity, bilateral, radiological S&I. However, some physicians do not identify within their dictated reports that the catheter was repositioned at the bifurcation. The report states that “the catheter was advanced from the common femoral access to the supra-renal aorta, and abdominal aortography with runoff
was performed.” This dictation only allows for one code 75630 (aortography, abdominal plus bilateral iliofemoral lower extremity, catheter, by serialography, radiological S&I.

Question:
Can code 75630, aortogram, be reported with a unilateral or bilateral extremity angiogram (75710 of 75716)?

Answer:
No. This would constitute double billing of the extremity angiograms. As shown below, code 75630 includes an aortogram and visualization and interpretation of bilateral lower extremity arteries via a run-of. For this code, a catheter is advanced to the infra-renal aorta and, without moving the catheter farther down the aorta or in one of both extremities, a run-off of contrast provides imaging of the both extremities to include the iliac and femoral arteries.

Code 75625 is for an aortogram, only. Code 75710 or 75716 is reported in addition to 75625 if the catheter is moved to the aorta bifurcation or into one of both extremities, providing additional imaging of one or both legs.
