VITAL STATISTIC REQUEST FORM

Please complete this form if you wish to receive a certified copy of a BIRTH CERTIFICATE, DEATH TRANSCRIPT and CERTIFIED TRANSCRIPT for MARRIAGE/CIVIL UNION RECORD. This only applies for vital statistics events occurring in Orange (births, deaths, or marriages/civil unions) in Orange, New Jersey. In addition, the following information must be provided:

One (1) Photo ID with address or Two (2) alternate forms such as:

• Non-photo Driver's License • Vehicle Registration
• Insurance Card • Passport
• Green Card • County ID
• School ID • Utility Bill(s)

PLEASE NOTE: The individual requesting birth certificate(s)
or death transcript(s) must be one of the following:

*Subject Subject's Parent
• Insurance Card • Passport
• Green Card • County ID
• School ID • Utility Bill(s)

Once completed, please return the form to the Department of Vital Statistics along with $15.00 per document requested.  Enclose a self-addressed stamped envelope.  After receiving this information, the Health Office will get the certified copy out in approximately one week.  Make the money order out to:

Marriage/License Information

The marriage or civil union license must be obtained from the registrar in the New Jersey municipality in which either applicant resides, if one or both is a resident of New Jersey. If neither applicant lives in New Jersey, they must apply in the town in which they will be married.

A witness, who is at least 18 years of age and knows both of the applicants, must appear with the couple at the time of the application.  He/she must have I.D. The couple must bring their birth certificates and second form of I.D.  The birth certificate must be in English. If either of the applicants have been married before, they must bring their original final divorce decree or a certified death certificate.

There is a mandatory 72-hour waiting period between the time the application is made and the issue date.  This includes weekends.  Once the license is issued, it is valid for 30 days within the State of New Jersey.

The cost of a marriage/civil union license is $28.00.  Exact Change is preferred.  Marriage applicants are taken between the hours of 8:30 a.m.and 4:00 p.m.  Monday through Friday, unless otherwise noted.


Birth, Marriage/Civil Union, or Death Certificates occurring in Orange are available for $15.00 (cash or money orders, or personal checks) each through the Health Office.

The Bureau of Vital Statistics is available for Marriage License Applications by appointment only between the hours of 9:00 am – 12:00 pm and 2:00 pm – 4:00 pm. Please call 973-325-4121 to schedule an appointment.  Vital Record Copies can be obtained Monday - Friday from 10:00 am - 12:00 pm and 2:00 pm - 4:00 pm.

About the Bureau of Vital Statistics

The Bureau of Vital Statistics is conducted under the supervision of the Township Registrar. The Registrar is required to see that all births, marriages, deaths and civil unions take place within West Orange borders are properly recorded locally and with the State Vital Statistic Office in Trenton. Certified copies of vital statistics records, marriage licenses, and burial permits are issued by this division. Census track mortality statistics are also kept on file.
